I’ve had an email and a letter about the 3rd vaccine (which I’ve had) and now about the 4th vaccine. It says that “ I may have a medical condition or be receiving treatment that could significantly weaken my immune system” and may now need a 4th booster dose.
I’m only on 4mg pred but have been on various doses for 11 years. There are other conditions I’m being investigated for including Polycythaemia Vera and Adrenal insufficiency....I don’t know what has triggered the letters.
My query is that the letter seems to imply that I need to take a referral letter from my 3rd primary dose or a letter from my doctor. As the letters I have say I “may” need another dose, do I need another letter to take?

I don't know the answer to that question, but I would be moving quickly to make sure I got it. Contact GP asap or is their a contact number of email on the letter.
I have just received the letter from NHS. I had my 4th jab 2 weeks ago by taking a letter from my GP surgery. Not sure if I should. Have had it as my dose was 7mg then but grabbed the chance and surgery happy to provide letter. Go for it
The 3rd dose letter will do it - you mean the one that says 'you are receiving treatment that may significantly weaken your immune system...'? I took that as instructed on Gov.uk to the nearest walk-in and it was fine. AS it was an ordinary pharmacy, I phone ahead to make sure. It is still less your current dose that matters than what you were on with the earlier jabs. But if there are grounds for your doctors to suspect polycythaemia vera, that would put you at higher risk of clots if you caught Covid. SoI would think the extra jab would be advisable - and your letter should get you in at a walk-in centre, provided you are over 91 days since the third jab. They won't do it earlier.
This is a generic letter sent to many of us it would seem. Very wooly and vague. I have mentioned before that I have been a volunteer vaccinators since September 2021. The screen questions ask if you are immunosuppressed. Once you answer "yes" you should get the jab without any further ado. Unfortunately the NHS computer systems don't seem to 'talk' to each other. I had my 4th jab one month ago but received a letter like yours yesterday inviting me to have number 4 or ignore the letter if I'd like I'd already had it! Thereby covering all bases but another waste of NHS resources. I'm considering writing to the Minister for Health not that it will do any good I'm sure.
