I wish I could help. I had/have endo stage 4. Hysterectomy done in stages. Still have bad period type pains. I have had a 8 ops. 2 since my last ovary removed. I had an mri with contrast and that showed frozen pelvis. That is most likely causing my pain. I am due in for another mri because the pain hasn't gone. Diverticulosis showed up on my ct recently. You could maybe look into both of those?

So sorry you are still in pain. I had a hyst and bi-lateral ovariectomy years ago. I was put on hormone replacement bc i was only 31. The pain came back. I stopped the hormones and the pain became less. However there were still areas of endo embedded in my utero-sacral ligaments and my right round ligament. I saw an endo expert (now retired) who did a scope to check my pelvis and biwel. He removed those 3 ligaments and I have not had endo pain since.

I would caution that since you have had a hyst, caution should be taken re ligaments. Some of the ligaments may have been used to suspend your bladder, vagina and/or rectum. But it is possible there is still endo on the peritineum - which can be removed without issues. There are still plenty of hiding places for endo. Find a doc who knows that.

But first - stop taking hormones (if you are now) and give it 6 months. The only hormones you MUST have is a small dab of estrogen cream on the outside to keep your kady oarts from atrophying.

Endo is a life long fight. I have been in it for 50 years now. It is survivable. Best of luck...

Do you have any pelvic floor PTs in your area? Sometimes they can help the pain with muscle release / myo-fascial treatment!
