Im writing this after a long journey of pain in pelvic floor. Im really desperate and it affects my life badly. Maybe i will get some answers from you guys.
First, Im a male 31 years old. It all started a year ago, one day after work i felt like i had difficulties of holding the pee. It was not the worst feeling but it just was uncomfortable. After few days it went away. After a week it came back with burning symptoms and pains in the perineum. I went to 2 urologists , did all tests(including semen test, ultrasound) and nothing came up - no infection, no stones, nothing. The pains are coming back and going away in terms of days ( 3 days of pains and then 3 days of no pain at all ) - its not a static pain. somedays i dont have pain at all.
I tired to find a pattern for the pain and found out a strong correlation to ejaculation. Almost every time I ejaculate, immediately after it i have burning sensation and feeling uncomfortable. Sometimes it goes away after a day, and sometimes i have pain for few days which includes urine issues and burning in penis. Every time I prevent any erection or ejaculation for a week - I feel really good.
I went to a physiotherapist of male pelvic floor and did stretches for almost a month. But yet i cant tell of a significant improvement.
Did anyone else had this issue as well ? That he has pain for days because of ejaculation ?
I hope to get some useful information because its a really hard time for me.

You probably have some neuromuscular stuff going on there I've had pelvic floor dysfunction for two and a half years so I joined the pelvic floor dysfunction group on Facebook and you might want to join the Pudental neuralgia group on Facebook too both men and women have these issues.... I ended up going to a pain doctor if I went to physical pelvic floor therapy for a year and a half we realized it was some neurological stuff going on with the muscles and nerves... I have found Gabapentin to be helpful very low dose and also just started cymbalta too low dose both help nerve pain and there are more options too but as much as I didn't want to take meds I am glad I did because it helped my quality of life now
Thank you for the response.The reason I dont think I have a nerve issue is because of two things. First , as I said it has a correlation to ejaculation. If I prevent it for up to few days I'm starting feeling good with no pains at all. Also I have heard that nerves include numbness which i dont have. I think it's a muscle thing. But the stretches doesn't help my symptoms and I'm doing it for a month. Also the PT evaluated my pelvic tension and said it's not tight at all
Helloeworld, I would not rule out a problem with the pudendal nerve. One of its branches runs to the tip of the penis. I have probable irritation of the rectal branch of the nerve when I have a bowel movement. If I did not have a bowel movement I would have less pain (I say less because sitting usually aggravates the nerve if you have a problem). With that thinking, ejaculation might be irritating the branch that runs to the tip of the penis. Just a thought.
I reckon you should keep on with the yoga stretches and make it part of your daily routine as well as pelvic floor exercises 3 times a day.It will take time.Keep at it! Worth it 👍😘
I have pudendal neuralgia and although I'm female I can relate to your burning pain. I just wanted to let you know that nerve pain doesn't always include numbness. It can also cause burning pain or itchiness. Lyrica and cymbalta have helped with my pain. Pudendalhope.info has a lot of information on pudendal nerve pain with lists of therapists and doctors. Hello, I am a female (so hope you don't mind my 2 cents worth of opinion).
The pudendal nerve is a major nerve on the right and left sides of our body. The pudendal nerve controls our bowel, bladder and sexual functions. This nerve branches out to smaller nerves to these areas. The best way to rule out pudendal neuralgia is to get a nerve block. Ask for the long acting block, because you have a specific pain that needs a longer time to evaluate. In my experience the block I got to verify pudendal neuralgia worked for 4 hours. Later, they gave me a block that lasted a month at the pain center I go to. They use steroids for these blocks. Nerve meds don't work for me either. I have had the best response with botox shots and radiofrequency ablation of my pudendal nerve. I hope this helps.
