I have had chronic inflammation/pain in my pelvis for around 5 years. The condition has appeared to manifest as an ache in the pelvis (mainly rectum) that appears less than 24 hours after an orgasm, and typically lasts 4-5 days. The discomfort prevents me from sleeping, so I have to take an anti-inflammatory just to sleep. Urology wasn't a help, physical therapy provides some relief. Taking long breaks from orgasm has proven to remove the pain, but is not a sustainable solution for my life.
My muscles are very tight & I'm working on that.
Does anyone have any insight on this case?

Try seeing a womens specialist physio who could do vaginal trigger point therapy, it helps to relieve tight pelvic floor muscles.Hope you find a treatment that works well for you.
Sound like you have tried Physical Therapy and you might need to do it more often to retrain the muscles in the area. You can learn to consciously relax your pelvic muscles during the day and reduce the stress in the area by just being aware sometimes. Your PT should be able to train to do self PT regularly.
