Just an update. I had a proctogram examination today and it has confirmed pelvic floor dysfunction. I'm a 44 year old man and have been suffering for years with pelvic pain which was made worse after pooping. I'd suffered constipation and developed haemorrhoids from straining to defecate. It turns out that even when pushing down to defecate, my internal anal sphincter is not opening or not opening fully at times and so I have not been evacuating all of the stool within my system, causing it to back up and cause constipation and pain. I am to be referred to a colo-rectal surgeon and need to have biofeedback training in order to learn how to re-train the muscles to work in synchronisation again. If anyone has had any success with this method of treatment I'd be grateful to hear your story. I hope you are all managing ok. Thanks.

I have almost same thing. I have had extensive pelvic floor physical therapy. I had biofeedback. I would recommend this, even though I am still struggling every day. I believe I'm not coordinating the muscles correctly when having a bm. Ny first bm in the morning is fairly easy, but will have another about an hour later and is difficult to get out. I have internal and external hemorrhoids from pushing hard to get it all out, but I feel I have incomplete evacuation. Try physical therapy. You have nothing to lose.
I hv the same issue in emptying .. first bm is easy or slightly comfortable but the second one anytime during the day leaves me in so much pain.. I also hv constipation issue as well as piles.. what physical therapy would u recommend or do I have to speak to my GP..
Hello my friend. Never feel alone, that's the main thing. I see a colo-rectal specialist shortly and the doctor who referred me recommends biofeedback therapy to train the muscles to relax when pooping. I think a combination of diet and therapy helps. I will let you know how it goes. Try making a banana and spinach smoothie in a blender with a bit of water in the bottle. It has helped me. Do see your Doctor and try to get a referral to a specialist. It sounds like anismus and pelvic floor dysfunction too. Best of luck.
I will definitely speak to my GP. Last time I hd consultation with colorectal specialist and he said its skin tags thats r causing all that pain.. I personally think its a combination of few things..
No problem at all. Make sure to ask your GP for a thorough pelvic floor examination and ask if you could be sent for MRI defecography. It's a very useful test. I had it and it has shown that my internal sphincter muscle is not working as it should. I feel this must be related to my pelvic pain. I too was found to have haemorrhoids when I went for a sigmoidoscopy examination. If there is anything you want to ask or share on here, please do. A pelvic floor Physical Therapist will work on myofascal release, check pelvic floor muscles to see if they are too tight or too weak. Also, could be non coordinated muscles working to push stool out. Biofeedback may also help. Try a Therawand which PT can also help with. I was also taking Gabapentin which I believed helped but I had side effects. My next step after seeing countless drs. is internal injection of steroid in the pudendal nerve. I'm on waiting list. This is just another guess. I am going back to PT again with different therapists as I think this is the gold standard of treatment. Good luck. Try all options. Never give up.
Your symptoms are very familiar to me. I have pelvic floor muscle dysfunction for over 30yrs. I still get issues before BM. Don’t push hard. This makes things much much worse down there. From experience get Pelvic muscle physical therapy. Your obturator muscle is way too tight and hinders defecation. You can get muscle injections to calm it down or also Botox injections. Peace
