Good morning! It is 4/6/2014 early on a Sunday morning hear in the USA. I am giving the reading of my last blood test. My ALP is 542.

My ALT is 97. My AST is 68. My Mitochondrib M2 Antibody is 116.8. I am taking Ursodiol 300 mg. two times a day. I will have my blood test taken again in two weeks I hope the numbers come down by then. It will let me know if the Meds are working. They are doing all kinds of blood work and will test for any vitamins that I should be taking. I will let you know the results.

Fingers crossed urso works for you.. My Alp was +1100 last may.. It's not in normal range as are my other liver enzymes (they were all high) . My alt is slightly elevated but I'm happy with how much urso has helped.. I take mine all at night xx

Hello 2006.

Back in 2010 the same day I had the LFTs taken along with the AMA for suspected PBC (was diagnosed Dec 2010) my ALP stood at 603. The 3 months prior it was 614 soit can fluctuate (I was on urso for either of these readings at the time). 3yrs on my with urso ALP rises and falls and though it has steadily come down in that time and 2 months ago on repeat LFTs it stood at 231. I think the highest normal figure on my reading is 130 so from over 600 to just over 200 is by far better.

You might find as I did that in the early days of urso you get quite dramatic drops in the LFTs as I did but then mine slowed down. But I am apparently now within a normal range for PBC (as the scale is different from someone without).

I find with PBC and taking urso you find it very hard to know exactly what the LFTs and GGT are going to be like each check. I can feel on top of the world and get slightly worse results than the last time and vice-versa. Don't despair, any drop is far better than none and it can take a year to actually see a big change. For me it has taken just over 3yrs for my night time itch to feel a lot better than it had been. In the early days I did suffer fatigue but at some point within the year of urso it vanished and I am not fatigued any longer. Do get tired at times due to the night time itch but fatigued not.

I was diagnosed two year ago with Stage 4 PBC. I am taking 1000 mgs. of urso/day. I also take Provigil for extreme fatigue. It is not working as well as it once did. Does anyone have any suggestions?

Are you saying the Provigil is not working as well as it used to?

I never heard of a med that would help w/the fatigue. In fact when I asked I was told there wasn't a med for that.

I am checking w/Dr. Google as soon as I am done here to see what he has to say about Provigil. If it did help you with the fatigue at all I am very interested in it.

Is it possible to increase the dose of your Provigil? Perhaps it's not as effective after some time on it...

In the beginning it worked great. Now I seem to fade at work at about 11:00 a.m. I also get to work by 7:00 a.m. since mornings I have more energy and that's when I get most of my work done. Remember I am Stage 4, it might work better on someone who is in a different stage. I can increase the dose but we had to appeal my insurance company's initial attempt at declining the prescription so I don't want to go through that again. Also there are risks as in anything you take since all goes through the liver but you have to weigh quality of life. I have a good job as a paralegal and cannot afford to lose my job because of the extreme fatigue so i opted to take it. It's worth discussing it with your doctor.
