A case study post bariatric surgery

For anyone who has access to twitter, this case study posted yesterday might be of some interest. Probably as much for the discussion as the original case.

(Don't get too excited! It is just another case where the origination of the problem doesn't get blamed. As I see it, the cause was iatrogenic and predictable - being a known risk. But it was the lack of follow-up that allowed the problem to develop rather than be treated.)

A 47 year old woman developed imbalance and falls 1 year after gastric bypass surgery. Good diet and takes a vitamin. Her cervical MRI is below

Here’s the twist: serum B12, ceruloplasmin, folate, and urine copper were all NORMAL. What’s going on here?!

On this forum we know that there is no such thing as 'normal ' B12. It varies wildly from person to person. When will doctors realise the same thing? The amount of misery caused by doctors rigid adherence to the B12 value range is disgraceful.

This is cool. i

This has triggered me to set up an appointment with this doc that posted this case on twitter for two reasons.

1. see what this guy has to say about b12 deficiency.

2. bring awareness to my case , other cases and results. if this doc is willing, also try and find a way to collaborate with him to help bring more awareness to this issue.

Mayo Clinic is in my network and many of you all have seen how I have pointed out that they let me down the first time I did a visit back in 2020; I had an official diagnosis, but as always was let down by being told that b12 wasn't my problem.

Perhaps I can ask if they can bring more awareness to patients of what type of specialist to see and if a patient goes to the wrong specialist, to get the awareness out there to get those specialists to refer to him rather than leaving the patients helpless like I was.

I'm totally dreaming here. But it doesn't hurt to try and i have the rare opportunity of MC being in my insurance despite that I am in UT.

If any of you have suggestions on what to bring to the table, pitch in.

Assuming he is willing to listen, What message do you want me to deliver????
