Hello Everyone,

I am having heart palpitation and fast heart rate from quite sometime and loose stools.

Done several tests (Full blood count, liver function etc) and ECGS all came back normal apart from b12 which was border line.

Last week did a private blood test (medichecks) for thyroid function along with other test (kidney function, full blood count, folate etc) to see my overall body function and results came back today. I just need a bit of help.

HAEMOGLOBIN 125g/l (130-170)

HAEMATOCRIT 0.36 L/L (0.38 - 0.50)

RED CELL COUNT 3.83 (4.4 - 5.8)

Mean cell volume 95 (81 - 98)

Folate 2.57 (>3-20)

B12 Active 28.6 pmol/l (37.5 - 188)

Vitamin D 15 nmol (50 - 175)

Can someone please explain what these results indicate. All of my other tests are normal.

It means that you are feeling terrible (probably worse than you realise at the moment because it has come on you gradually) due to a vitamin B12 and folate (vitamin B9) deficiency.

You need regular B12 injections and folic acid tablets from your Dr.

Hopefully if you take these results and show him/her the NHS guidance on the Internet for B12 and folate deficiency you will get the treatment you need immediately.

Any problems come back here and we will try to help you further.

Thank you for your reply. Intrinsic factor antibody test came back Negative.

Coeliac test also came back normal. Just saw my previous b12 results one on 2014 and one on 2017 both are border line low but my gp never told me about this.

GP just prescribed some B12 injections and I am going to have it this Thursday. I will also request my GP to prescribe me folic and Vitamin D tablets as well.

As deniseinmilden says, plus I think your Vitamin D is very low.

You’ll need folate to help B12 to work, too.

Hopefully oral supplements plus a course of B12 injections will help but maybe you need to consult a dr on why several things are low.

Thank you for your reply. I am going to get b12 injections (Hydroxocobalamin 1mg) this Thursday and will also buy folic acid tablet and vitamin D.

you may have a mix of folate, B12, iron and vitamin D deficiencies going on - suggesting some sort of absorption problem in the gut. the combination of folate/B12 and iron means that interpreting full blood count may be difficult as folate/B12 is associated with larger rounder red blood cells and iron deficiency is associated with smaller red blood cells so if the two are going on at the same time they can mask each other.

PA is a strong possibility but you should also be checked for coeliacs, crohns (given history of gut problems) SIBO and h pylori as possible causes.

Thank you for your email. All of my IRON tests are within normal range:

IRON 20.64 umol/L (5.8 - 34.5)

TIBC 51.94 umol/L (45 - 72)

UIBC 31.3 umol/L (22.3 - 61.7)

Transferrin Saturation 39.74% (20 - 50)

Ferritin 107 ug/L (30 - 400)

It is only B12, Vit D, and Folate which are low along with Red Blood cells. Intrinsic factor antibody test came back Negative and Coeliac test is normal. I have been tested for H pylori and it came back positive but I don't have any symptoms of H Pylori.

Please also see my full Red blood Cells:

HAEMOGLOBIN 125 g/l (130-170)

HAEMATOCRIT 0.36 L/L (0.38 - 0.50)

RED CELL COUNT 3.83 (4.4 - 5.8)

Mean cell volume 95 (81 - 98)

Mean cell hemoglobin 32.5pg (27 - 33)

MCHC 343g/L (300 - 350)

RDW 13.1% (11.5 - 14.4)

Are you being treated for h pylori? - would be a course of anti-biotics along with PPIs whilst on the course of antibiotics to allow any damage from h pylori time to heal.

According to this article it isn't uncommon to be asymptomatic with h pylori infection
