You need to ask.
1. When do I start my course of B12 injections.
2. Do I start with 400 ug a day of supermarket folic acid, or are you going to prescribe 5 mg folic acid tablets for the short term?
3. Can I please have ferrous fumarate rather than sulfate for iron supplements as the sulfate causes gut problems.
4. Can I have an Intrinsic Factor antibody test?
5. Do I need to see a gastroenterologist and/or neurologist (if you have any neurological symptoms like pins and needles).

overlap considerably with thyroid - but that doesn't mean that the symptoms are being caused by thyroid - your levels above - for folate and vitamin B12 are very low - particularly B12 ... and you may well have had these symptoms for a long time.
PA isn't the only thing that will cause a B12 absorption problem - you have multiple vitamin and mineral deficiencies - which PA can lead to - but could also be other things going on - so you might also want to look at coeliacs, h pylori infect ...
Have you ever had gastric surgery and do you have problems with acid reflux?
If the tests were done privately the GP may, unfortunately, want to redo them just to double check they are accurate.
