Did you photocopy your last set of forms , if you are the same just put that and add your back problem with any copies of letters from doctors confirming the problem.
You could re read the nras booklet to make sure you are still getting the same points across as your previous application plus the new ones.
Have a look on the Benefits and Work website, its not the diagnosis but how it affects your daily living. Send all relevant recent hospital correspondence and keep appointment letters etc. The onus is on you to give them a complete picture of the difficulties you have with daily living, they wont be checking with any of your doctors.
Hi, I hate these forms they are so stressful. Last time I took mine to the local Welfare Rights Dept they were really helpful and took a lot of the stress out of the process. Also nras do a booklet on applying for pip I think.
