Does the sperm come out of the penis only during ejaculation or does is come out all the time.?

Here I am not referring to pre-ejaculation fluid. In my understanding this fluid does not contain sperm itself but can have sperm from last ejaculation.

You’ll normally have precum when you get excited. Then it just produces that during your excitement. Which does contain sperm. It is coming from the same place.

That is how some ladies end up pregnant, even though a guy has pulled out to ejaculate.

The point of "pre-cum" is to "clean" the urethra from urine (it may contain sperm as well). Normally, ejaculation happens during orgasm, climax, however there are some cases when "pulling out" didn't work and a person did get pregnant.
