You will need to be far more specific with your question, if you seriously want helpful advice.
Why don't you book an appointment with your GP and talk your concerns through with him/her? If you do, spend some time in advance thinking about which areas of your health you wish to discuss. Don't be afraid to make a list of them so that when you are in the consulting room, you don't forget, or omit to mention, any relevant points.

That is a big question with no one answer. Best places to start would be improved diet and more exercise.
This does not have to be a big change you can still eat chocolate and bacon sandwiches, though not together ! Try by reducing sugary foods, and limiting alcohol intake. Exercise can start with walking a bit more, going swimming once a week. Little things add up. As Akshay1234 says it may be a good idea to speak to your Dr.
Nearly every teen experiences this, so you’re normal. You can try some weight training to muscle up a bit and tone, but in the end, to others, it doesn’t matter. Your value is not in your physiche.
