Possible Circumcision

I am 55 years old and have a long foreskin with long overhang even to the extent that when erect the glans isnt exposed. I love my foreskin and have always enjoyed the fact that it pulls back fully no problem. That is until a couple of months ago when out of the blue it was tight when erect and wouldn't pull back unless forced and then it was uncomfortable and became red and sore. When flaccid it still pulled back fully but did seem a little tighter even then. I went to the Dr who didnt know what it was and gave me some cream but nothing changed. I returned to a different Dr who explained that it can just happen and that he has known 70 year olds that suddenly get this for no reason. He prescribed some different stronger cream and said if it doesnt work then he will refer me to a urologist for a possible circumcision. Unfortunately it doesnt appear to be working and the cream is leaving its mark on my glans ie making it slightly red and patchy even though I only apply it thinly and avoid the glans. Only problem is when the foreskin is peeled back I cant avoid the cream going onto the head. So I fear the worst and really dont want to be cut. Has anyone else experienced this out of the blue. Apparently I dont have a condition like phimosis, it can just sometimes happen for no reason. Has anybody else experienced this and get referred to a Urologist? Could they help with any other treatment? I worry about how to show the problem to the urologist and didnt know whether to take a video of my erection demonstrating how difficult it is to retract the foreskin or would that seem weird ? If I end up having to have the op I then worry the stitches will split as it's rare I dont wake up without an erection too. So many worries and questions going through my head. I would apply the steroid cream quite thickly over the whole foreskin and glans and remember that you still need to stretch your foreskin. The cream will make it softer and more stretchier. When I did it my glans was red and sore due to the balanitis. It helped me (it also gave me more sensation in the glans which was good) . You can only use the steroid cream for a short time as it thins the skin (it will thicken up again) so you need to stretch quite hard even though it hurts a bit. No pain no gain as they say. You should ask to be referred to a dermatologist first instead of a urologist they are skin experts and may find the problem. The doctors urologist are very quick to suggest circumcision it’s quicker and easier for them as it removes the problem right away but it’s not them that will loose sensation and the pleasures of having a foreskin. I’m treating circumcision as a very last resort . If I have to then maybe I have no choice but I won’t be an easy decision.

Thank you and your reply makes sense albeit the Dr told me to apply the cream very thinly ! Maybe he has a target for circumcisions haha. It really is my last resort.

Ok apply it thinly but make sure you cover the whole area inner and outer foreskin too. But this will only soften the foreskin you still need to stretch. There are guys on here that have been circumcised and stretched their skin enough to recover the glans again so just shows that the skin should stretch.

I was told by the doctor as a kid to pinch the end shut when Taking a pee let the Foreskin expand like a balloon to stretch and have did it for over 65years.

Wow. I would end up with pee everywhere knowing my luck. I have certainly used that method to trap semen spurting everywhere. One of the advantages of being uncut I guess.

I would also say to pull your foreskin back when you pee in maybe ask for to be checked for diabetes.

Hi Man, do you have any scarring on the foreskin or the phimotic ring which is causing the tightness?

That good so if it was LS you would have some scarring. If it doesn't improve you could see a dermatologist then. but I'd keep going with the steroid cream and stretching it can take some time. It could also be a fungal infection and definitely get checked for diabetes it could be all linked.

Cheers for the reply MrMac. Def not a fungal infection as Dr has checked that, he says it's just one of those things that can happen and he has known it happen to 70 yr old guys who have had to be circumcised. Typical it would happen to me as I've always loved my foreskin, its length and flexibility. Really hoping a urologist can come up with a solution without circumcision needed but I'm sadly fearing the worst.

Ask to see a dermatologist before a urologist.

I seen a dermatologist. His diagnose was "Phimosis". And told me to see a urologist tomorrow for a circumcision. He had the attending nurse write me a script for "Circumcision". He commented It's not that painful".

Yeah shit happens that's life the body does change as we get older. keep going with the cream anyway maybe see dermatologist if it doesn't improve. If you do need a circumcision it's not that bad you will adapt sometime it's needed. 🙂

Ask yourself what colour it is. If it is porcelain white that is lichen sclerosus, the condition often known by the misnomer "balanitis xerotica obliterans (BXO)". If so it needs a high potency topical steroid. Hydrocortisone won't help. Clobetasol is most effective but medics are nervous about putting it on the script.

There is anecdotal evidence that a mixture of clotrimazol and hydrocortisone (Canistan Hydrocortisone) is effective, people have reported it works, but there are no clinical trials to support this (researchers anyone?). This treatment is available over the counter but pharmacists won't sell it if you say you are going to put it on the genitals!

If it is a different colour then the options will be different. Surgery need not be circumcision it could be, eg, multiple Y-V plasties. You will need to find a surgeon familiar with this and willing to do it.

Hi,

I have, and I'd definitely recommend you go and see the urologist to get an expert opinion! He's not just going to have a quick chat and then just whip it off with a stanley knife, so you'll have time to consider your options, whatever they recommend.

GP's have lots of knowledge, but they aren't penis experts - so take the appointment, a big list of questions and get the right answer for you.

My foreskin had to come off because of cancer, but I don't actually miss it! In fact, i think knowing what I know now I'd have had it done in my late teens.

OMG where do I start Jon. What a brilliantly written blog and its taken me most of the evening to read. I have had some laugh out loud moments which got me strange looks from my wife and I have had moments where I actually filled up reading about what you have been through. The bit that was relevant to me with the possibility of a circumcision has provided a lot of comfort and I thank you for that. However, my situation doesn't even come close to what you have been through. I agree with others that you should take to the stage and do public speaking on this topic, you are the man ! From this blog I am a type 1 phimosis and that has only recently happened. However, the blog doesn't tell me how to do stretching exercises on this type, only applicable if you have type 5 etc.

Try sitting in a nice hot bath then with an erection gently pull your foreskin back and stretch it gently but firmly.

I had a circ last August for mild LS. I'm over 60. Everything works fine for me and no loss of length. Depending on your diagnosis, if you need to have the op for medical reasons you can read up on this here regarding where to go and what type of circ you can have.

I have minor tightness that has developed through my 60s - possibly because I don’t have sex any more and masturbate less often. However I find coconut oil applied night and morning helps a lot.
