Up until recent cuts in services and consequently my redundancy, part of my job, ironically, involved supporting people with mental health problems back into work. My folk frequently experienced problems with their ESA claims.
I have also experienced problems with missing paperwork throughout my own claim. In my case, missing medical information sent by my GP. My advice would be to keep close track of your claim at each stage. Check that they have recieved relevant documents and remind them of the timeline in which they are meant to operate. If you do encounter further problems you can always make an appointment at your local jobcentre or citizens advice bureau. A benefits advisor or DEA (disability employment advisor) can be a useful advocate.
Current ESA administration appears to be creating much anxiety, stress and, in many cases, financial hardship which we could certainly do without.

always photo copy the item so that you dont have to start from scratch and try your local mp ask him to get on the case of the missing claim paperwork its amazing how when a politicain gets involved, or another agencey like CAB people seem to respond a bit more. good luck
suggest you google NCIL- you will be able to find your local centre for indep living- they will be able to advise you-+ help- they filled in my DLA form for me+ when i was rejected they got me a lawyer + wrote the appeal- which eventually went to a tribunal- which i won-in fact without having to attend- they just rang up on the day + said the judge had found in my favour! so don't give up- just get help! also i had probs with ESA- was granted it then had a medical assessment- by a 'medical professional' who rejected the letter from GP+ consultant+ said i'd be ready 2 go back 2 work in a few weeks!! i was really shocked + rang up - + a really nice guy told me that that they were rejecting everyone + that i just had to put it in writing etc etc- anyway- i appealed- that was rejected but i persisted- i was waiting to hear the tribunal date when they wrote saying they had 'reviewed' my case + found in my favour!! so they are just trying it on to see who will give up- so keep going- you KNOW you are genuine + so do your doctors- even if it seems to take forever- [1 year for DLA, 9mths for ESA in my case]- it's worth it and they will back date the money.

I worked as a generalist advisor for the CAB for 3 years before having to give it up because of Lupus - having said that - if anyone has any problems claiming benefits - especially things like DLA and ESA make sure you have the help of your local CAB. They are trained to write the words (they have to be the right words, in the right order for the government to take note) and if you don't succeed the first time, they will appeal on your behalf. They are all volunteers and have your best interests at heart - not the governments - completely impartial.
E re myven if you don't go thru CAB, one tip is that you log ALL phone calls, date, time, always get the name of who you spoke to and what was said. When it comes to appeal (and you should go to appeal), the fact that everything is logged goes in your favour.
