Research study looking at transition from child to adult clinics for Rheumatology patients

This study from The University of Surrey is hoping to collect patient perspectives on their transition from child to adult clinics and how this can affect them and their condition.

If you are aged 18-30 years old, were diagnosed as a child with a rheumatologic condition and have been through some sort of transition to an adult clinic in the UK, then please take 5-10 minutes to complete this survey. All responses are anonymous.
