I always use Freedom insurance - they cover all our APS preconditions.

We used ‘Travel insurance 4 medical’ they cover my husbands APS and they cover him even though he’s had a previous sudden cardiac arrest. We took out a yearly worldwide (incl USA) travel policy for our family of 4 and I thought it was very reasonable.
I use More Than - I imagine it changes depending on the specific situation of your pre existing conditions but I’ve had no problems with them

I tend to use EuroTunnel these days, mostly because I am using them for travel too, but I don't think you have to, and they do do annual cover (I don't, because of things constantly changing, my financial adviser agrees "insure the trip, that way you know exactly what you are covering"). I have also used tour-operator / travel-agent policies for the same reasons, but these seem to be getting more selective. EuroTunnel ask more or less three questions - are you travelling against drs advice, are you travelling to get medical treatment, are you terminal - and I try to stick with providers who ask the minimum. Once you start on detailed medical questions then (a) it takes age to get a quote (or no quote), and (b) you get ambiguous questions with multiple choices where multiple answers could apply (or I do) - for me that is a red flag.
Remember good travel insurance is not necessarily the cheapest, but one that pays out when you are unconscious in ICU, not one that refuses and leaves your relatives trying to crowd-fund your life support because one medical question could have been answered either way. You hope you never find out whether you bought good insurance or not. Shopping around for the cheapest travel insurance may be ok when you are in perfect health, but not when you have chronic complex conditions - in my opinion.
Travel insurance is a nightmare several companies refused to insure me but after last year realised travel insurance is a must. In the end I got a decent quote from SAGA, I do realise that some of you may be under 50! Also, whilst researching insurance I learnt that the WI do insurance as long as you are a member and under 75 and will cover all medical conditions. Good quotes too.
Unusually for me I did phone SAGA, I generally do everything on the net, spoke to an advisor which was much easier than trying to fill a form in.

Whatever my trip is, I got to insuremytrip.com for insurance. Very good site for comparing policies and deciding what is the best policy to fit your needs.
Just purchased two policies for two different trips I have planned. Word of advice: make sure to purchase a policy within the required time frame of booking your trip, there are some restrictions/rules regarding this depending upon the trip itself.
I use Freedom and have always found them reasonable.
