I've recently bought cover with Coverwise. Very reasonable though hasn't been put to the test by making a claim.
I'm in the US and travel frequently but here and abroad. I use Travel Guard Gold. And I make sure to order the insurance as soon as I book the trip so pre-existing conditions are covered. I also am a member of Medic Alert Travel Plus, which, among other things, covers medical evacuation up to $100,000. Because I often travel overseas, I'm also a member of IAMAT, which has a vetted list of English-speaking doctors available in emergencies all over the world. Hope this helps.
Hi - I am here in the UK and I recently gravelled to Croatia and I used Insure and Go. I declared I had APS (Hughes Syndrome) and of course the price went up but I was covered. Hope that helps.

I got a reasonable quote from Freedom Insurance, to go to France for a week, they were very helpful. Their details are on Foundation website.
I went through money supermarket and although expensive £480 odd - I have annual worldwide insurance - going to Cuba next week and LA in July, (from UK) so all in all worthwhile - insure and go were a bit more expensive - all declared (APS, PEs, Dabigatran) good luck! ( rest of the family -4,off we're £130 for annual worldwide - not very fair but eh - it's good cover! X
