Getting your medical record….

, if you have managed to get medical records and whilst tracing back throughout been alarmed of random diagnosis of what you had not been the mental health team psychiatrist , being able to write a record , based upon just a telephone call , once every three months.?

From your post it is slightly unclear about what you exactly mean. From what I understand you have been given a diagnosis which you were unaware of concerning your mental health. You need to speak with your GP and find out exactly why it was put on your records. It could be a simple coding error. Have you been given medication in regard to the diagnosis.

How do you get your medical records please?

Hi Tedy, I got mine by filling out a form at the GP's and received an email link to it about a month later.

Hi Mia. Thank you for your reply. Does the form have a number and do you know how far back it goe s?

Hi Tedy, I'm not sure but think it's just called a 'subject access request' form and doesn't have a number. Mine was a bit of mess to be honest as I've recently moved and changed GP's who are on a different system to the old ones, but it did have some things in there that were over 20 years old. Hope this helps.
