I'm not sure to be honest.
My son's written me a letter for my PIP review and he's put what I can't do, how things affect me if I try and do them and what would happen if he didn't help me with different things etc. It was checked and okayed by Welfare Rights who filled in my review form for me in December.
Know how you feeel! I am helping my younger sister to getPIP at present, we are at Court of Appeal stage so far.. I wrote to her Consultants for back-up, her g.p for backup I asked them not to be positive on anything about her conditions as the DWP home in on that. We copied sick notes, prescriptions for oromorph, things like that and all have been sent in to Court of Appeal for further reconsideration. You would probably send to DWP if you are not at appeal stage. It seems they dont care what you suffer from they need to know how illness affects you each day in great detail.
Best thing is to tell what you see with this person. Do they walk do they fall over are they slow on mobility. ? Give examples of what you observe which is how assessments are done. Be honest has the person got worse since you first knew them and how? just pointers . Remember the buzz words of not realiably or safely!
Oh type your letter so no mistakes like me I cannot spell!
