ESA?

Morning folks!!! I will have been on ESA for one year in June and I was wondering what happens when it ends? Can I ask to be put in the support group? Do I have to have another assessment? I'm not too worried about it at the moment but I know as the time draws closer I'll be stressing!! I had to fight soohh hard to get it in the first place but won my appeal in January!! Any advice will be gratefully appreciated... Much love.....ninja!..
