Maturity issue

Firstly, huge respect to anyone going through this process. I know first hand just how hard this all is.I'm currently 5dp5dt (transferred a good quality B grade blast and a 5 day mourla - fresh transfer). Trying to stay positive but have a gut feeling that it hasn't worked this time. Testinng out the trigger and still a vvf line this morning.

This is our second round of ivf.

For clarity, I have low AMH 4.2 (uk), DOR, 7 AF count, Diagnosis of Adenomyosis (although new Dr said she couldn't see signs) and suspected Endo - Yay!

Before commencing treatment (ivf) my Dr suggested a laparoscapy. However, due to the wait time of circa 11months, we opted for a HyCoSy instead. Our fear was time wasn't on our side, I'm 39 and I had read a lap can effect overian reserve. Tubes appeared clear.

Ivf Cycle one - meriofert 375 per day. Stimmed for 6 days and was told to trigger that night. Clearly overcooked. At retrieval, 3 eggs retrieved, 2 immature. One ICSI'd (no sperm issue but wanted to be safe with only the one). This resulted in our high quality A grade blast. Fresh transfer but didn't implant.

Second cycle - meriofert 300 per day. Stimmed for 12 days. 6 eggs retrieved, 2 mature. 1 x grade B blast and one mourla at day 5. Both transfered Fresh.

Ovitrelle trigger used both times. I asked my Dr for a double trigger before the second round was dismissed.

On top of all of this, we've been trying for over 6 years and not once have I had a positive test (excluding post trigger). Even before that, we have never been overly careful and have been together for over 20 years. No chemical, no miscarriage, nothing. Given even some 'poor' embryos can implant, I'm convinced there is an implantation issue going on too.

Sorry for such a long post, I just need any help and advice I can get. I'm so scared that this will never happen for us 😭

I don’t have any specific advice but I just wanted to say that I’m sending all my positive vibes to you - remember it only takes one. Keep looking after yourself, it’s a hard journey and you’ve been through a lot. Hoping you’ll get that BFP soon xx

I am sorry 😞 . Are they triggering you too early? Are your follicles growing in sync?

I didn’t have any proper implantation until I had immune protocol and lap excision of endo. Have you and your partner had karyotypes done? Has your partner had sperm DNA frag checked?

Lap shouldn’t affect ovarian reserve unless you’re needing endometrioma addressed.

Thank yon so much for your reply. I think they may have triggered too early, especially given my prior maturity issues. I had 13, 15, 15, 16 17, 18, 19 and a lead at 24! I assume my two mature ones were the 18 and 19. I'm looking into immune issues, will pursue the laparoscapy and ask about the sperm dna fragmentation test. Husbands sperm sample was supposedly 'excellent'so they never mentioned this to us. We only used icso as I had such low numbers and they wanted to ensure fertilisation. I'm so happy that the immune protocol worked for you ✨️😊🙏

I felt like it was me writing this post! I’m also 39, low AMH and AFC in the UK. First round 375 meriofert, most eggs were immature on day 14 retrieval. I got 1 day 3 embryo, which didn’t implant. Next round they put me on BC and 450 meriofert but my body didn’t respond, so it was cancelled. 3rd round they said they’d do ICSI. I had 12 eggs retrieved on day 14 but 0 were mature. I’m currently on day 11 of my new round with gonal f and pergoveris. This time my dr said I would stim for 14 days and do a double trigger as I just can’t seem to get any mature eggs. However they don’t do EC of a weekend, so they’re saying Friday might be too early (day 14) but Monday might be too late (day 17). So feel like I’m in the same position AGAIN. I too have endo but due to NHS waiting times, it will be years waiting for a lap! You should follow DR Victory. He’s a Canadian dr that has a specific endo protocol. The problem with endo is that we’re more likely to produce immature eggs. He also says that if you’ve got endo you should never do a fresh transfer as you need to let the endo dampen down after all the stim drugs have raised estrogen levels . Praying that this works for you, but if not, hope some of these ideas help. xx

Thank you so much for your reply, our stories really are so similar. I'm very sorry to hear you are going through such similar issues, the feeling of being told how many eggs were immature is not something I'd wish on anyone.I'm so grateful for my two rounds on the NHS, but it did feel a bit a 'one size fits all'. Looking back, I wish I had challenged my Dr over the double trigger, but you live and learn.

I understand your frustration, my clinic also didn't do weekend EC. What sizes have your follicles been at trigger. Last time I had 13, 2x15, 16, 17, 18, 19 and a lead at 24. I'm sure the lead was lost and the 13 was empty. My collection was on a Friday, but looking at the sizes, I wonder if they could have pushed me to Monday as I'm guessing my two mature eggs were the 18 and 19. I feel they should have learnt this from last time but alas.

I asked about frozen, but the NHS protocol is fresh and then frozen if anything left (that never happens). I did raise this but again, one size fits all.

I'm pretty sure my period is on its way, day 6p5dt and stark negative. So I will be scheduling a follow up with my consultant. My next questions will be what further testing is available on the NHS for implantation failure and can I be put on the laparoscapy waiting list ASAP and prey there is a cancellation spot available! I also have a small fibroid, which they said is small and not an issue, but I'm trying to cover all bases!

I'll look into Dr Victory, I'll take any help and advice I can get! I'm just feeling so defeated at the moment and would give anything to see a positive result. This journey is really wearing me down, I though this one was the one 😢

Please keep me updated as to your journey, I'll share anything my consultant tells me in the hope it may help you. I'm just not too sure what to do now and feel in limbo, do I carry on with private ivf or get the lap first. I've even considered paying for the laparoscapy, but gosh it's super expensive and will steal from our ivf piggy bank. I guess I'll need to wait and see how long the wait time is. I wish you all the luck in the world and hope dearly that you get your BFP on your upcoming round! Xx
