It sounds like you've covered quite a few things, but maybe take a look at the tests recommended in this webinar for getting IVF ready as well - youtube.com/watch?v=oaQLqny...
Did your husband take the antibiotics you took for gardnerella and ureaplasma too? We discovered gardnerella in my vaginal microbiome a month ago and the gynaecologist prescribed it to both of us. We also did a sperm culture test (which probably should be accompanied by a urine culture test) to leave no stone unturned in my husband as well.
Ahead of the second cycle, our consultant referred my husband to a fertility urologist to re-check his varicocele (probably good to do that if the DNA frag test came back lower than desired) and he also recommended karyotype testing, but that was based on one test where the sperm count came back lower than usual.
One thing I'm considering doing if the next transfer doesn't work for us is a Thrombophilia (clotting) screen to check if I have "sticky" blood.
That's all I can think of right now, but the tests recommended are usually guided by the IVF process and the points at which the eggs / sperm / embryo didn't perform as well.
