PICSI or ZYMOT? also Intralipid or IVIG?

Good Morning everyone, I hope you are all well and having a good start to the week.

I am hoping someone can add some valuable points or thoughts to help on our next and final round of ICSi.

We have had 6 rounds of ICSI and the closest we got to a BFP, sadly ended up an ectopic on round 5. This round was on short protocol with the NK treatment of intralipid, steriods and inhixia.

Round 6 didn’t pass the 2WW so we want to give the last round a final and good shot, throwing everything and the kitchen sink in to an extent, before having to call it a day.

I am looking at two clincs, one offers ZYMOT the other PICSI, do you know which is better please?

Also, regarding intralipid, is IViG or Humira meant to better, especially in terms of treating cyckotines?

Evaluating these 2 points will really help us decide which clinic to go with, our current one or one in London. As expected, information out there is limited for us all so I’m using this forum as my sounding board.

HiI don't know anything about PICSI and ZYMOT I'm afraid but I am seeing a immune specialist at the moment who talked about intralipids, ivig and humira.

I had high TNF Alpha (cytokines) and the initial suggestion was for me to have humira alongside hydroxychloriquine. I personally was not comfortable with humira so I asked for alternatives. At that stage I was given the option of IVIG or intralipid. The suggestion was to start with intralipid and see if that reduced the cytokines. Sorry I don't know if this helps. I guess what I'm trying to say is that my doctors preferred treatment for high cytokines appeared to be humira but I felt this wasn't for me however it is supposed to be effective at reducing cytokines xx

Is there a reason why you didn’t prefer humira?

I went on the humira website and was put off by the Important Safety Information message that comes up on the home page. It was my personal preference after understanding the side effects / risks to try something else first xx

hi Skittles, sorry to jump in here but may I ask if the intralipids helped lower the cytokines? I’m in the exact same situation with a high TNF Alpha but don’t wish to take humira as suggested, so was hoping for alternatives. Many thanks xx

Hello. In my case the intralipids did not help lower the cytokines. I think it varies so much from person to person. The other thing that strikes me is that the doctor wanted me to re-test after taking intralipids but before any transfer to see if the TNF alpha had improved. But I don't think most people do that kind of re-test. They just take the intralipids or ivig or whatever it is, alongside the transfer. Are you seeing a reproductive immunologist?

hello, thanks so much for your reply. That’s helpful to know, and yes I’m seeing a reproductive immunologist and he’s also said I should retest after the humira injections, which I’m refusing to take. So was hoping for an alternative which I guess I can discuss when I see him next.

my current clinic did say they don’t use humira and IViG due to concerns, so I’m wondering if anyone has had success with these if I do go with them for pICSi / zymot.

hi have you used PGS embryos or untested embryos? I’m wondering if that may change the outcome.

I appreciate not everyone tests though. I had a consultation today and was told that IVIG is stronger than humaria and the intralipids. If the other didn’t work then they’d jump to ivig for your next protocol. So maybe best to Junp to it for the next transfer x

Hi There, we’ve just had a failed round with PICSI and IVIG. The PICSI resulted in 4 good quality blastocyst’s (we didn’t test them so they will say the failure occurred due to Chromosomal Issues I’m sure).

The IVIG is incredibly expensive (over £2k Vs £600 intralipids). Unfortunately it has had an impact on my liver function in terms of the enzymes (they test the liver enzymes before and then during the TWW). I’ve just had a repeat test to hope it’s all gone back to normal.

Not sure what clinic you are with - I will PM you with further details.
