Low AMH Long Protocol

Hello everyone,

I wondered if I could please get some advice. I've recently just had my first round of ICSI (January 2022) using short protocol.

During this round we found out that one of my ovaries didn't respond at all to the injections and my consultant thinks this ovary is unlikely to produce any follicles/eggs at all due to blood supply issues.

My left ovary had 8 follicles but only 2 eggs were collected. Unfortunately neither egg collected fertilised which we were so upset by.

We also found out I have low AMH for my age. I'm 28 and my AMH is 2.5 pmol/L.

I'm now going into my second round of ICSI. This time the consultant has put me on a long protocol with injections starting 14th February and egg collection, if all goes to plan, scheduled in for 7th March.

I've had a look online and seen on many websites that a short protocol is better for someone with low AMH. I was wondering if any of you have low AMH/diminished ovarian reserve and been put on a long protocol? If so, how did you find it and was it successful?

Should I be worried I'm on a long protocol?

Any help will be so appreciated.

Hi, my AMH was 1.6 last April and my first round was long protocol. I managed 2 eggs on collection, only one fertilised and made it to day 3 to be transferred, unfortunately never took. Second round was short protocol and I got 4 eggs and 2 made it to day 3 transfer. One stuck but unfortunately lost at 5 weeks. Short seemed to work better for me, but I have heard from ladies with low AMH having success on long.

In regards to your worry, I am hoping that your clinic wouldn't put you on something that would not offer you the best chance of success. However, If you are concerned, perhaps speak to them, as you need to feel confident as much as you can going into the treatment. Sorry, I couldn't offer you much advice.

Hello,I have extremely low AMH/diminished ovarian reserve for my age and I'm responding well to long protocol. I was on long protocol 3 times already (sadly, last one cancelled just before an eg collection as I got Covid) and 1st time they collected 7 eggs all 7 fertilised, 5 made it to 5 day blasts and 1 is my son, 2nd time - 4 eggs, 3 fertilized and 2 made it to 5 day blats, and this time it looked like I would have around 6 eggs.

I would say give it a go as you really didn't respond well to a short cycle. Long one is tiring but hopefully will give you a baby :)

Hello lovely, same as the previous poster. I think it depends case to case. I have heard/read that long protocol, the downregulation part of it can no good for low AMH people. My first doctor was only using short protocol for me (twice), but was giving me a trigger that shut my hormones just like the down regulation would, so technically my third round was long protocol, being immediatelly after the last trigger. My base hormone levels were definitely much better as if I got younger :) The third protocol was with a doctor who specialises in low ovarian reserve. He wanted to put me on long protocol but then checking the base levels said to start stimming straight away as me hormones were already at a level as if I had downregulation already. He also said that long protocol can produce more eggs but of lower quality. Which in the end was the result for me. More eggs but compared to my first round with least eggs, the useful embryo count was lower, most fertilised eggs just stopped growing at an early stage.

I think you jusz have to try what feels/works better for you. Good luck 🤞

The clinic take into account your history blood results etc and then tailor the treatment protocol that is best suited for you If still unsure about your regime ask the clinic to explain it to you againBest wishes for the future

hi lovely, I'm low amh diminished ovarian reserve. I did short protocol, didn't work for me. my next one they are downregulatung me, so they have full control. I would trust them, as everyone is so different. they will have learnt from your first. Good luck xx
