Pregnancy with large endometrioma

Am interested in trying to conceive in the next year. was found to have a 10 cm endometrioma on my left ovary confirmed by MRI. I have pain during my period, some bloating and constipation but no other symptoms.

I am wondering if it really is best to have it removed - worried about the effect on my fertility. but not sure how safe it will be to potentially be pregnant with an endometrioma of this size. but i have heard of successful pregnancies with cysts of this size, having a scheduled c section and removing it at that time. any advice/input/insight appreciated!

Hello

As far as I'm aware, for anything bigger than 4 or 5 cm the recommendation is to remove it. For natural conception, removing it is thought to improve chances of success as well

Good luck with whatever you decide to do

if it’s that large then removal maybe best. The pressure of the cyst could damage your ovary or even cause ovarian torsion and then you could lose an ovary which could impact on fertility massively. I’d speak to your gyny specialist and see the pros and cons of surgery vs no surgery. Good luck xx

Definitely at that size get it removed , it could leak out and cause you to have you ovary removed.

I had one at that size and the pain , it had my bent over, unfortunately I lost that ovary not sure whether it was the size or what x

I was pregnant last year and they found two cysts 8cm and 10cm on my left ovary during a scan which they assumed were just simple cysts. They couldn't do anything but leave the cysts due to be being pregnant at the time but no-one seemed concerned and my midwife said she knew of someone having a 23cm cyst removed at the same time as a cesarean at full term. Personally I was a nervous wreck with this but unfortunately that twin pregnancy wasn't meant to be. My cysts did then resolve by themselves without treatment or pain thankfully but I was initially planning to have them removed surgically prior to TTC again.

Fast forward 6 months later to my next pregnancy and scan, this unfortunaly ended up being an ectopic pregnancy as my left fallopian tube was kinked and endo was found during my salpingectomy. I suspect the cysts may have had something to do wrecking my fallopian tube but no-ones ever explained fully to me and my endo was widespread but they didn't do any endo removal during surgery.

So like you, I'm TTC and wondering if further surgeries are help or hindrance. My recurrent miscarriage consultant recommended avoiding further surgery's and try to conceive ASAP but that's easy for him to say..

Thank you for sharing your experience. I am sorry to hear about the difficult time you have had. Like you I also do not feel confident with either choice, surgery or not. It seems like neither are really great options and its about choosing the lesser of two evils 😔

I had a 10cm removed from my ovary last year, the gynae said they wouldn't leave it as it was very large. I've been told I was lucky to have one the most skilled surgeons, as he managed to drain and remove the cyst skin without damaging my ovary at all. Post surgery I've been told they wouldn't do the surgery again as it can cause damage and affect fertility. Unfortunately, I have been unable to fall pregnant regardless of the surgery. But that could actually be down bad timing or whatever lol.
