Possible unilateral endometrioma question

Hi everyone,I had fertility testing as we are trying to conceive. I'm 41 and wanted to know whether all was ok. All was clear with HSG, hysteroscopy where a small polyp was removed. The only thing they found was a 2cm cyst on my right ovary that was sugestive of endometrioma. Fast forward to March I found I was pregnant but sadly miscarried at 8 weeks. I've been worrying about this possible endometrioma and whilst getting an US to confirm miscarriage last week, I asked them to check for my ovary to see whether the cyst had evolved. It was still there. A week later (today) to check if mc was complete, they found out that the cyst appearance completely changed, it wasn't homogeneous anymore and had structure inside. Not sure whether I am describing it well. A week ago they wrote in the report that it was suggestive of endometrioma but this time, they didn't.

I am trying to make sense of it all. Can endometrioma totally change appearance in a week? Could it actually be another kind of cyst?

Just as an aside, I don't have any typical endometriosis symptoms and this cyst was an incidental finding last year.

Any help or insight would be much appreciated!

Hi

I'm so sorry to read about your mc.

I'm not too sure about cysts and how quickly they can change but I was diagnosed a few weeks ago with suspected 6cm endometrioma on 2 seperate scans, but I saw gynae yesterday and they are doing more investigation to try and work out the cyst type as she's not 100% that it is an endometrioma. Mine has a solid component inside.

It's frustrating because both reports suggested endometrioma I had in my head that's what it was but now it feels like a back step.

All gynae could confirm is that it's not a simple cyst, it's complex and warrants more investigation to come up with a plan.

Hope you get sorted soon

Have you ever been diagnosed with endometriosis? I would love for my doctor to tell me it's not an endometrioma. At my age I'm damned if I have it removed and damned if I don't fertility wise. I have never been diagnosed with endo nor have I had symptoms and that scares me.

Never formally diagnosed. Dr made an assumption based on the endometrioma which may not be an endometrioma. Gynae consultant is questioning 'endometrioma' as it has some irregularities but I have had symptoms of endo for years. Heavy periods, immense period pain and continued bleeding with mirena and after ablation, also anaemic more than not since age of 15.So I guess in a sense I'm back to square 1 and trying to ascertain a diagnosis of some sort whether it is endo or cancer or something else, I should know more in next few weeks.

I have undergone lots of blood tests, biopsies and scans over last few weeks. I understand you would love for your Dr to tell you its not an endometrioma but in my case the less likely an endometrioma the more likely cancer. They are discussing my file on Tuesday in an MDT as its complex.I hope yours turns out OK and that they can sort out what's going on relatively easily and I really hope you get the outcome your hoping to achieve and that fertility is on your side once a plan is in place.

Have you ever been referred for laparoscopy? If you have had strong symptoms for years they should do the due diligence. In any case I hope that your cysts are beningn, at the end of the day that's the most important thing. I will see an endo specialist and have another scan with the NHS with another specialist and hope I can get to the bottom of it. It's hard not to overthink it.

I have never been referred for laparoscopy but I led a life believing my symptoms were normal I women that have periods so I never made a noise about it until 2 years ago when my body physically expelled 3 coils in a few months, following that I had an ablation but again I was naive and didn't push for answers. I have now been told my ablation has failed and I guess they may investigate for endo now 🤷‍♀️

I wish you all the best and hope that in your case these are endometriomas indeed. We always have to advocate for our own health, otherwise not much happens sadly.
