I think your sugar levels may be considered satisfactory considering that you are a diabetic since 27 years and so must be advanced in age.I also think that ,keeping your cardiac issues,you should just follow your doctor's advise.What is your HbA1c ?If it is 7 or less,you have nothing to worry.

Looking at your sugar levels, I think you need to talk to your doctor to split the insulin / medicines and add afternoon dosage as well to cover up lunch. Sugar level after lunch and before dinner are high, which means your sugar level from Lunch to Dinner is always high which affects HbA1C as well.
