Consult your diabetologist. Anti-diabetic medications have to be prescribed with reference to your present sugar level (FBS & PPBS), HbA1c, age, weight, Blood pressure, etc.

As makrim suggested start a low carb diet sure your doctor will have you follow. gangaharan suggested all the test you need done to be evaluated. This is not a funny disease your numbers are very high you run a risk of losing limbs. At least get an evaluation see where you are at then look at your options.
