" Abstract

Purpose: Parkinson disease (PD) medications are not readily available in all countries. Citicoline increases dopamine synthesis and inhibits dopamine uptake. This systematic review aims to synthesize current existing evidence on the efficacy of citicoline adjunctive therapy in improving PD symptoms.

Methods: An extensive literature search of Scopus, Embase, PubMed, Cochrane Library, and Google Scholar was conducted for articles published on or before December 31, 2019. The studies were screened and selected by 2 independent reviewers. We included all studies that explored the efficacy of citicoline as an adjunct therapy in PD.

Findings: A total of 7 studies (2 crossover, 3 randomized controlled, and 2 open prospective studies) were included. Despite the varied outcome tools, this review found that patients with PD who were taking citicoline had significant improvement in rigidity, akinesia, tremor, handwriting, and speech. Citicoline allowed effective reduction of levodopa by up to 50%. Significant improvement in cognitive status evaluation was also noted with citicoline adjunctive therapy.

Implications: Citicoline adjuvant therapy has beneficial effects as an adjuvant therapy in patients with PD. However, due to the heterogeneity of the studies, there is a need for more high-quality studies."

I have been taking it for the past thirty days and dont notice any improvement. Maybe I have to take it for a longer period.

Hello House2 ~ On 1/3/2022 I started my HwP on Acetylcholine (ALPHA GPC with Bacopa). Last week I felt like there were improvements. Nothing that I could really put my finger on, just a generalized improvement. When I asked him about his "inside" jitters he told me they were kind of on and off. I was going to switch brands of this but have now decided not to change for the time being.

I have over recent days added 4 x 250mg citicoline (taken alongside Sinemet) to my mum's regimen. She was beginning to struggle with freezing, increased shuffling, general physical incapacity. Getting to the loo was becoming a significant ordeal; in my absence she would have fallen. She's 82 and uses a three-wheeled walker about the house.

I've been hugely surprised by her rapid improvement. She's much more physically capable. Once corners have been negotiated, she is now capable of a strong, lengthy stride that doesn't look like a Parkinson's stride. It's as if the clock has been turned back 30+ months. Cognition is more difficult to assess (she has vascular dementia + likely Alzheimer's), but she seems a little more present.

No sign of dyskinesia, of dopamine excess (so far).

I'm delighted.

Should add that, to my mind, her Parkinson's has always been a little atypical:

No obvious on/ off issues...just a slow degradation if a dose of Sinemet has been forgotten. And, she sleeps beautifully (10hrs straight through, night after night).

Hi Garygjs, Does your mother still take the same dose (1000mg a day) and it is still working for her?

She now takes 500/750mg...somewhat driven by my concerns about her ability to swallow large tablets and a broader concern about not overdoing anything (even while citicoline has a great safety profile.)

I mix a 250/500mg into yogurt twice a day...and, honestly, she is 83, has vascular dementia too and there's a limit to how much yogurt I'm happy to ram down her throat on a daily basis. She has a lot more besides citicoline mixed into yogurt and eating it is not a thing she much enjoys.

Her gait, her walking capacity has slowly deteriorated since that initial - frankly magical - transformation but she is still walking better than just prior to citicoline's introduction. That slow deterioration began while she was still on 1000mg.

So, she is still walking better than 12 ... 15 months ago. Not at all bad given her age and conditions.
