Financial Assistance for Patients with Chronic Lymphocytic Leukemia (CLL) & Small Lymphocytic Lymphoma (SLL) lls.org/support-resources/f...

If you are diagnosed with CLL or SLL, you may qualify for The Leukemia & Lymphoma Society's Co-Pay Assistance Program, which provides eligible patients up to $4,500 annually for treatment-related expenses.

If you are eligible, you can use your award toward any or all of the following expenses related to your CLL/SLL diagnosis. Please note, assistance is not restricted to treatment expenses, so patients on a watch and wait protocol should also apply.

Covered Expenses:

• Medical insurance premiums (Medicare and Medicaid included)

• Treatment-related co-pays, deductibles, and co-insurance (for expenses covered by the Co-Pay program)

• Co-pays for prescription medication related to prescribed treatment

• Co-pays for labs, scans, and tests as follow-up care after being diagnosed.

LLS can pay your provider directly or reimburse you for payments already made.

Based upon historical and recent trends, we anticipate that the CLL Co-Pay fund will be open through the winter of 2025.

For a full list of covered expenses, visit our website at LLS.org/copay.

Yes unfortunately:To be eligible for Co-Pay Assistance, you must

Be a United States citizen or permanent resident and reside in the U.S. or a U.S. territory.

Members of the U.S. military, diplomats, etc. who may be serving overseas, must have a U.S. address and use it on the application.

Have a household income that is at or below 600 percent of the U.S. federal poverty guidelines as adjusted by the Cost of Living Index (COLI). You will be asked for your zip code to determine your COLI.

Have medical insurance

Have a blood cancer diagnosis confirmed by a doctor

The patient must be in active treatment, scheduled to begin treatment, or is being monitored by their doctor.

For members of the U.S. military, diplomats, etc. who may be serving overseas, the healthcare provider attesting to the diagnosis must have a National Provider Identification (NPI) number and use it on the application.

Sounds like an excellent service but fortunately co-pay doesn’t impact us in the U.K.

For U.K. patients struggling to cope financially, Macmillan can be extremely helpful. In certain situations they can approve grants but their main function is in helping patients to maximise and access benefits etc.

I am blessed as I have a patient assistance program (PAN) that pays for my Calquence. The costs of these drugs are far from reach of most people.
