after bendamustine got covid

hi all after use bendamustine my numbers went down but unfortunately i got civid what should i do please help me thanks for sharing your thoughts

Hi mazaheri, sorry to hear you’ve contracted Covid and hope you’re not feeling too unwell. I see you live in Iran and I’m afraid I have little knowledge of your health systems there or protocol for dealing with Covid. However, I think you should inform your Consultant and ask for advice. I’m afraid I don’t know if there is availability of therapeutics there. Main thing is watch for a rising temperature and a drop in your oxygen saturation levels.

hi nedawn thanks for your reply yes lm in iran l was in hospital 9 days for covid now lm pretty well l got 2 unit BLOOD for going up my numbers. wbc is 10.1 hgb is 8.1 crp is 50 must be up to 6 i dont know after covid when can I enable to complete my remission.as you know after bendamustine must use rithuximap

i didn't have enough power for continuing for chemotherapy. pls explain what should l do?

Oh this must come from the guidance of your specialist mazaheri. I understand you have an oncologist? You need his/her clinical guidance on how to stage your CLL treatment following Covid but I’m pleased to hear you are now doing well after hospitalisation. Are you able to contact your oncologist for advice?

hi yes my oncologist said after covid will be stabled take blood tests cbc come to hospital for cll treatment and chemotherapy but l not well with this because first step of chemotherapy botherd me and now l haven't enough power for continuing treatments

Rest. Monitor your blood oxygen level with a oximeter. Take painkillers and something to reduce fever if you have it. If your blood oxygen level drops below 92%, call an ambulance. Nobody knows when and to what extent you will recover from covid. Unfortunately there is such a thing as long covid. Some people have lasting consequences. Not necessarily you though.

Mazaheri, it looks like you may be over the worst of COVID, so here are some ideas for you to ask your docs about relating to the short, medium and long term:1) Can you get COVID vaccines or boosters? Any chance you can get either Moderna or Pfizer? they will provide more protection than the J&J shot.

2) Are any of the monoclonal antibodies available in Iran? Some of them may provide some protection from another hospital stay, depending on which strain of COVID attacked you and may be lurking there. Here is a link to explain more about the MABs--lls.org/sites/default/files...

There is some evidence the MABs may also protect you from long COVID, too.

3) Be aware that Rituximab may damage your body's ability to make antibodies from COVID vaccines. The Leukemia and Lymphoma society has extensive research on this. It turns out that Rituximab (and other CD20 agents) damage B cells to the point that they can't make antibodies. here is a link with more info for you to share with your docs--lls.org/news/new-study-show...

You and your docs should focus on the timing of any future CLL treatment, to avoid problems with any COVID treatments.

4) if you can get N95 masks, make them a routine part of any indoor visits.

Good advice, with the important correction that the reason anti-CD20 monoclonal antibodies inhibit the effectiveness of vaccinations for so long, is because they remain in circulation in the blood for up to a year after your last infusion. So basically you won't have any health new B cells available to make new antibodies until the rituximab, obinutuzumab or ofatumumab has all gone from attaching to and killing new B cells as well as any surviving CLL cells. Hopefully there's some T cell response to vaccinations, but this is less well studied with CLL.

You're welcome. i'm fine. i've been in remission from CLL (uMRD) for 17 months and counting. And avoided COVID, too! Very blessed.

Hello mazaheri

I am somewhat older than you and have had bendamustine treatment as well. I had covid in December 2021 and did not have to go to hospital as doctor treated me over phone. Sick about 1 and one half weeks which included case of thrush from antibiotic. I completed all six of my bendamustine treatments several years before getting covid. Your general health seems to be good enough to get through covid, so your doctor should be able to advise you on continuation of bendamustine. Blessing and best going forward.
