Treatment of My Father suffering from CLL with Endoxan (Cyclophosphamide) and Bendit--Bendamustine Hydrochloride.

My father was diagnosed CLL- Chronic Lymphocytic Leukemia on 03-Sep-2013 through a bone marrow.

The values of %LYM = 80 % with WBC= 511,000.

We have consulted an oncologist for the treatment. He has started Chemotherapy with Endoxan (Cyclophosphamide) and did 06 Cycles of chemo (Each after 21-Days).

After that we have performed test from Hospital as advised by the Doctor and %LYM values reduced / down to 53 % as per attached reports of CBC and B2M. Sir, these reports are drawn after 06 Cycles of Endoxan(Cyclophosphamide).

The Oncologist started treatment with Bendit—Bendamustine Hydrochloride (as the image is attached) 03 X100 mg in two days 150 mg per day and same dose in second cycle of Chemo after 21-days.

(The reports are attached after the 01 Injection of Bendamustine and Second Dose for your review).

After applying two injection of Bendamustine HCL, my father is now feeling weakness and tiredness and we are poor people to afford this medicine. The doctor has asked us to repeat two injection of Bendamustine HCL.

Now, Sir it is requested to please advise further that if we will have to buy these injections or other treatment and there is any dangerous stage of Blood Cancer of my Father.

An Early response from your side will highly be appreciated to reduce my Tension my family as well.

Now the values of LYMP is in range 37.8 %

The decreasing LYMPH% shows that your father's treatment is working. A better way to measure improvement is to monitor the Absolute Lymphocyte Count. If that is not provided in the blood test results, you can work that out by multiplying the LYMPH% by the WBC.

Weakness and tiredness are common side effects from treatment, but your father should improve. His doctor should be providing you with much more guidance on when your father should be given his treatments and how many more he he should have to get the maximum benefit from them.

I presume you are in India?

The usual combination is bendamustine and rituximab, which in India is called Reditux, but bendamustine can be used alone.

The drug fludarabine is combined with cyclophosphamide and to these Rituximab is added known as FC or FCR which might be an alternative treatment option. Fludarabine and Rituximab, FR can also be combined.

I strongly suggest you consult another doctor for a second opinion at a major cancer centre...if possible.

Sir, First 06 Cycles with Endoxan- cyclophosphamide and later two Injection / Cycles of bendamustine HCL {each in 02 days(150 ml in day-1 & 150ml in Day-02) after the gap of 21-Days}

Nadeemnaz, I appreciate that you are trying to do the best that you can for your father under difficult circumstances. While members of this community can (and do) share their similar experiences and provide helpful suggestions that can be discussed with your father's doctor, even those medically trained are unable to provide appropriate medical advice without having a full picture of your father's condition. That is part of the reason why there is the following disclaimer on the right hand side of every HealthUnlocked web page:

You will do the best for your father's health (including appropriate treatment for his CLL), by finding a knowledgeable doctor in your own country. CLLcanada's recommendation that "you consult another doctor for a second opinion at a major cancer centre...if possible", is excellent advice.

Absolute Lymphocyte Count value become 3.175 (WBC *LYMP% = 8.4*37.8)

That's great news. Your father's lymphocytes have fallen from an extremely high level to just into the normal range. Ideally, he should also be eventually examined to see that his lymph nodes have shrunk and that the lymphocytes have been reduced in his spleen and other internal organs that may have been infiltrated, as well as in his bone marrow.

Further on why it is so important for your father to be examined by a specialist doctor; it is often things that we don't know about, but can be determined by examination by someone with the relevant training and experience, that can be critical in providing life saving treatment (or delaying or ceasing treatment for that matter). Without medical training, you wouldn't know what to look for (and hence be able to tell someone via email, etc) and without a complete picture, it could be very dangerous for your father's health for medical advice to be provided on the limited information you provided.
