A fatty liver can be reversed in most cases - it involves stopping any source of attack on the liver so cutting out alcohol is a must and if your diet and exercise levels arn't great then they too need improving.
The liver itself has no pain receptors BUT an enlarged liver can impact on the Glisson's Capsule which surrounds it and many people with fatty liver describe having 'liver pain'. I don't know about the liver twitching sensation - it may or may not be linked.
Key thing is some major healthy life style changes and hopefully you can reverse the fatty liver and symptoms

The twitching is caused by your enlarged liver pressing on things it shouldn't. My (first) liver was massive and experienced it too.
If it’s only fatty, it can be reversed. But do yourself a favour, stop the drinking, at least until your liver is happy. Ideally even then as you will may be more susceptible to injury should you carry on.
