Former drinker. So I have noticed some red blemishes on my upper back. They are small but are not going away. I can't really see any blood vessels branching out from the blemish. Does spider nevi due to cirrhosis usually occur on the back? I have about three of them.
I had slightly high liver enzymes and have had a ct scan, liver ultrasound, and fibroscan (5.4 kpa 180 cap). I feel like I have symptoms of cirrhosis such as insomnia, weakness, and internal tremors. My most recent blood test showed normal platelets and ALT 48 (11-61 range) and AST 38 (17-54 range). I had an EGD and colonoscopy as well.

Without the wee legs resembling a spider yours are probably Cherry Angioma which arn't liver related rather than Spider Naevi which sometimes can be. All your tests indicate no major liver issues.
Ask your GP to check them if you are so concerned, there are any number of types of totally innocent skin lesions.
From your picture they most certainly do not have any sort of resemblence to typical spider naevi.
The ones my hubby has are inches across with a very obvious central spot (body) and numerous thin blood vessels (legs) sprouting from them. Very much like the attached picture.
