Venlafaxine capsules v slow release capsules - side effects?

Ive been on venlafaxine for more than 15 years. I was moved to slow release capsules that seemed fine dose was 300. Then i went in to hospital and due to their stock they have been switching between tablets and capsule and vice versa. Also increased dose to 375. Three days ago i started to pee blood, the hospital thought a water infection i think its the tablets verses capsules. Can anyone please provide an educated answer to this i have read many articles. one saying the slow release capsules are strong should not take more than 225. The tablets only stay in the body for 12 hours so need to take twice a day. the capsules 24 hours. To verify go to bnf but i cant because i do not have a medical back ground. Please help i need to resolve this myself because the nurses are not helping.
