I'm working to overcome this myself.
For example, today I went to a bar where a musicial event was being held. I was pretty nervous going in, but forced myself to go in and chatted with the person next to me and listened to some music. (Actually, it was a little overstimulating because the woman beside me would not stop talking but Im SO GLAD I went just to face my demons )
My plan is to do this once a week, while continue playing guitar and writing at home. When I am ready, I will play a song at an open mic.
Everything is done in steps. It makes me sickly nervous but I'm doing it anyway. It helps that I see a therapist once every 1-2 weeks and I promise her that I will accomplish this. (Keeps me accountable)

I agree, you just do it and every time gets better. I was scared to lecture when I was teaching nursing school. It did get easier in time. I also get like that during surgeries but I just go in confident and do it. Good luck.
I was told to look over the top of the audiences heads, and pretend they were faceless, not to make eye contact and remember my lines...easier said than done, I think practice and confidence in your abilities will overcome most of your stage fright, but that just takes time....even if you do have to fake it.
I try not to be in the company of more than two or three people at a time
