HelpMe, May I suggest you are approaching your problems the wrong way. You have been battling anxiety, depression and low esteem without success. Why continue with strategies that have failed to bring you success? Time to move on.
You are full of self criticism, I think you are doing yourself a great injustice. I can see from your previous postings that you have courage, intelligence and strength. Lesser men would have been broken long ago, but not you. We are all given virtues to help us get through life, these are yours. Now use those strengths to rebuild your mental well being, to move forward and regain your self-respect. There is absolutely no reason you cannot walk tall and head up.
I'm sorry your high anxiety continues to trouble you. But with understanding, reassurance and a plan for recovery you can regain your quiet mind.
Understand that for reasons known only to yourself you must have been through a prolong period of stress: your anxiety is a perfectly normal reaction to this.
Be reassured that anxiety is a toothless tiger: it can frighten you half to death but it is not life threatening, cannot disable you or make you lose your mind. The power of anxiety is limited. It's a neurosis not a psychosis.
Take the tightness in your chest. It's a very common symptom of anxiety caused by muscular tension. My wife gets it whenever she is stressed. For years it was misdiagnosed a costochondritis. The stomach is the organ of the body most likely to respond to anxiety. Hence your GERD. They are not organ-based illnesses, they are symptoms of anxiety. They are caused by the fact that your nervous system has become over sensitised by stress and fear. You can spend a lot of time trying to heal these symptomatic illnesses but as they are not real organic illness I doubt if you will succeed.
Instead of trying to heal yourself of the symptoms, I suggest your time will be better spent healing yourself of their cause: high anxiety. When you overcome the high anxiety and your nerves lose their over sensitivity all those symptoms will resolve.
So far so good. But how would I suggest you employ a different approach to achieve recovery?
Stop fighting your anxiety - fighting only causes more stress and tension.
Instead, frame your mind to ACCEPT for the time being all the symptoms of anxiety. Agree to coexist with them for the moment. Carry on your life as normal as fearlessly as you can. After all you now have the understanding and reassurance given earlier on in this reply. Continue to accept rather than fight the bad feelings. As a result you will stop flooding your nervous system with fear hormone and it will gradually lose its sensitivity. Not tomorrow. Or the day after. But in the fullness of time for great things are not easily won.
And that's it HelpMe. That's your plan for recovery, along with the willingness to neutralise those pressures that have caused your anxiety disorder in the first place. Be ruthless to protect yourself if necessary.
You won't feel this bad for ever. You will recover, you have within you everything you need to recover. I wish you God's speed in your quest to regain your peace of mind.

This is a common thing that people with anxiety do. I hope after leaving the ER you'll get therapy you're money would be better used that way. Take care
