Meds not working

Need advice meds not working. I’ve been on Zoloft for 20 years....I also take Wellbutrin and lamictal. I only take 25mg of Z. If I stop taking it I start having anxiety attacks (prob withdrawals). If I increase I start pulling my hair (trichotillomania)...so I have to leave the Zoloft there. But I feel like 90% of my day is spent worrying. I cry a lot. Just basically depressed. I have a new doc bc my old one retired. A couple of times when I mentioned adjusting anything bc of fatigue she says things like “just stop taking the ...). You don’t need to taper that”. Scares the crap out of me bc I am afraid if I ask about switching to something new she is going to do something drastic and then I’m going to end up in the hospital. Anyone have any thoughts on what I could try? Or anyone have any experience with Zoloft not working anymore and switching to something else? Thanks everyone for being here
