Whether i hv bipolar or depression or Anxiety

I hv been taking following medicines.

Lithosun 400mg SR 1-0-1

Lerlift 100mg 1-0-0

Quitipine 200mg 0-0-1

Alprox 0.5mg 0-0-1

the above medicines i am taking for the past 12 years. Now i dont know whether i have bipolar disorder or depression or Anxiety. When i asked every year my psychiatric doctor, he said take medicines tegularly and dont bother about all. Any one tell me which one i have
