It never fails I sleep in 4 hour increments. As soon as I wake up I gotta do a checklist, which consist of checking my oxygen and temperature. Which of course just leaves me more anxious waiting.

Is there a medical reason for you to check these things? If not, doing specific tasks will cause you to wake up completely, and you'll have to start falling asleep again from square one. The best thing to do is roll over and try to go back to sleep immediately, before you lose your drowsiness.
I usually wake up at least once a night to hit the bathroom, but then it's back in the sack right away. It almost always works.
