At what point is it a concern after treatment .. ? I had LD brachytherapy a year ago followed by external beam radiation while having hormone therapy for 9 months. Subsequent PSA tests at 3 month intervals have been .01, .03, then .24. Due for an update test in a month or so but having a prostate still there I’d expect a rise as testosterone has started to return. I’m aware of the various studies < .20 survival rates and potential nadir spikes. I think I may have masterbated 2 days before the test .. Just a bit concerned but will know more soon. I’m wondering if anyone was in the same boat with the tri modality and expected psa levels afterwards. I’m ready to pull the PSMA test if another rise occurs (maybe +.50) hoping this is just a spike or? I mean what is the expected psa level with someone who still has a prostate and is in their 50s with recovering testosterone post ADT? The only answer I get from a non English speaking Japanese urologist is greater than 2.0 is an issue. Anyway any feedback or info appreciated..happy holidays to all,

it seems to be standard info that masturbation AND riding a bike close to a PSA test can make the numbers go up ... so, if you are scheduled for a PSA test ... refrain from these activities until after the test

As long as your PSA stays below 2.0, you're fine.

I guess I’m wondering expected PSA scores after such treatment non surgery with ADT. Seems like a lot of variables, doubling time, consecutive rises, Nadir, but with ADT assuming a spike from .001 after testosterone returns. From the perspective of doctors here .. low PSA undetectable or <.20 is good and once you get to 2.0 it’s an issue. Trying to be proactive so assume wait for next test and numbers and potentially do a PSMA pet scan between .5-1 for clarity. I just don’t see a lot of data on non surgical treatment with brachytherapy, linac, and ADT and expected positive outcomes in regards to PSA scores at year 1,2,3…etc. Would it be acceptable to have a PSA score under 2 for a long time as the prostate is still there or is it expected to stay much lower as most of the prostate has been damaged with radiation rendering it non functional. Regardless best to all.
