Article on when/how to take prednisone/prednisolone

I was waiting for breakfast to have with food. According to this it is not necessary and should be taken on waking especially for replacement doses.

And according to this it is not necessary to protect the stomach for most people.

(They do sell a supplement but is is not pushed very hard)

Interesting. But I see that it is based on only 1 healthy volunteer subject. Doesn’t that seem unusual for a study?

Of course but the main takeaway for me is to try to mimic natural cortisol, and so it is better to take it earlier. I was taking abi at 06:00 and pred 2 hours later (and forcing myself to eat). 8 for breakfast is way too early for me.

So now will probably take pred at 05:00 (when I usually wake)

Ok so I have been doing this for nearly a week and no issues (for me)

06:00 (approx), Glass of water, 120 mg Orgovyx, 1000mg Abi, 5mg pred, another couple of glasses of water. (I drink about 750ml but that is mostly because I am thirsty).

The idea is:

1 Enough water to dilute the prednisolone to avoid stomach issues.

2 Enough volume to encourage gastric emptying quickly

3 I need the water anyway (I don't drink much in the evening for the obvious reasons 🚽 )

4 It gets it all out of the way at once.

But I recently found out the Prednisolone I take is "enteric coated" which is designed to delay release until the small intestine (so the water is probably not necessary).

Maybe it is all enteric coated these days, but if not ask for it.

I am considering splitting them up and having a 30/60 min gap (safer?).

Because prednisolone causes a lot of stomach problems for me , I’ve experimented with taking it at different times before, after, during meals. When taking it right after a meal, it seems least troublesome. And yes, I use a stomach protector (Omeprazol) with my daily dose of oxy (which when not on the predni never caused stomach problems).

Interesting. Did you get stomach problems from the beginning or after a time? I tried it this morning (I only take 5mg) with just water and so far so good. What sort of symptoms did you get?

What bothers me taking it without food is not so much the acute symptoms but if it is causing longer term damage (perhaps with no symptoms).

Right from the start... pain in stomach and bowels. And acid reflux mainly during the night.

My MO has recently reduced the dosage from twice a day 5 mg to only once a day , this helps a bit. Hopefully just 6 more weeks until the current cycle of chemo has ended.

I just got my second batch of prednisolone and asked the pharmacist if I need to take it with food and she said it wasn't important because it was enteric coated which is meant to delay the release until the small intestine avoiding stomach issues.

If you are not already on it, maybe ask for it next time?
