Pluvicto + Keytruda + Xtandi

I’m being considered for a pluvivto + xtandi + keytruda trial. any opinions will be greatly appreciated.

You can join if you:

"Have received at least one prior line of taxane chemotherapy, or are unfit for, or refuse taxane chemotherapy. Note: Taxane chemotherapy administered in the Castration Sensitive Prostate Cancer (CSPC) or Castration Resistant Prostate Cancer (CRPC) setting is allowed.

Prior progression on at least one second generation androgen signaling inhibitor including abiraterone, apalutamide, darolutamide, and/or enzalutamide."

The previous phase I trial concluded: "A single priming dose of 177Lu-PSMA-617 followed by pembrolizumab maintenance was safe and had encouraging preliminary activity in patients with metastatic castration-resistant prostate cancer." thelancet.com/journals/lano... Therefore I would join this trial.

The idea behind it is that Xtandi increases PSMA expression at first. The radiation kills prostate cancer cells so that the immunotherapy may work better. I think Provenge would stand a better chance of working than Keytruda, but why not try it?

I would agree with Tall Allen: you may want to consider Provenge. 6-7 years ago, I didn't seem to be doing too well. My mo decided for me to try Provenge. At that time I was also on Xtandi and Zytiga. Getting approval for both drugs was not easy. Anyway, I was on those medications for about 11 months and, since that time I haven't needed to take anything, other than Avodart. The effects of Provenge don't appear on the blood tests, but the therapy seemed to work for me.

Hi Setho,

I have been on Xtandi for 2 months. I take all 4 pills at once every morning. No adverse affects. The first month PSA jumped fro .9 to 1.4 but my MO says it needs more time. I exercise a lot and I am hoping this will help any side effects. So far so good.
