Looking for 2nd opinion at Duke re new bladder cancer

Zytiga + Prednisone + Lupron have been successfully holding back bone mets since November 2021. Had cystoscope January 2023 to prepare for TURP. Bladder looked completely healthy, no cancer according to urologist. Then when TURP was done February 14, urologist discovered a tumor at bladder neck that came back positive for urothelial (bladder) cancer!

So he decided to wait six weeks for TURP to heal, then do another TURP on April 12 to look at bladder. Four days before the TURP, I had to go to ER for complete blockage of urine. I had a stricture at tip of penis, and ER staff could not get a catheter inserted. So urologist was called in, and she poked wires through the stricture (most painful thing I've experienced in a long time) and fitted me with a cath and Foley. So on April 12, the TURP resulted in finding that my bladder is quite filled with cancer, muscle wall involved! From zero on January 16 to full-fledged muscle invasion on April 12.

I am scheduled to consult with my MO next Wednesday, April 19 about what next steps are. Urologist said probably six weeks chemo then "probably" remove bladder.

I think it's time for a second opinion, and a friend with similar prostate cancer is happy with someone at Duke.

Any suggestions?

I'm so sorry to hear this. I've read some info that those who have bladder cancer are more likely to have other cancers too. I hope the doctors at Duke can manage it.

Just saw Dr. Daniel George on Friday. He is following me for BCR with 2 lymph mets. BTW, my wife is being treated for meta bladder cancer by Dr. Jonathan Rosenberg at MSK. She sits in my visits with Dr. George. Good idea to pay attention to anything TA advises. I concur--see Dr. George.

I have no real wisdom on this, but an anecdote that may encourage you. My father in law had bladder cancer and his bladder was removed. In spite of this he had mets to his liver lungs and back bone. He then had chemo, which ended up killing all the cancer. He lived another 10 years and died at home of a heart attack. Strong quality of life, golfing several times a week right up to the end.

A urologist friend says the chemo for bladder cancer is easily tolerated and can actually cure the patient, vs PCa chemo which can only slow things down.

Your situation will certainly be different, but I wish you the best possible result. God bless you.

Thank you so much! Encouraging stories mean so much, as I'm sure you realize. I have heard some similar experiences from people I know, also. I trust in God, doctors, treatments, and myself. All work together for good!

wishing you the best, surprised that they did not find anything in Jan but full blown bladder cancer in April?

Was there any sign or symptom during the three months?

I had difficulty urinating, which is why I had a TURP on February 24. That was supposed to be just "cleaning the pipes", but tumor was discovered at bladder neck. Apparently, urologist didn't see any other tumors, but scheduled second TURP (or TURB) to go back in bladder and look around. That's when he found other tumors. I am in a difficult place, because according to my oncologist, my prostate cancer has become castration resistant and I need chemo for both bladder cancer and prostate. Can't do both at same time. So I have contacted Duke for opinion and waiting on response.
