DX in 2020 with Stage 4 PCa and have done full course of Chemo treatment. Have been on Abiraterone for over 18 month but now it seems to be failing as PSA is continually rising; now 18 (was 2.39 back in April 2022) . I'm now mCRPC and still taking Abiraterone, ADT and Zometa.
Signed up for new trial sponsored by Bayer Co. Some participants will get course of Radium223 others will get course of Enzalutamide. Just found out I have been assigned to the Radium223 group
I have some bone mets (pelvic and L3 area) but no bone pain. Is the Radium223 treatment a good option for me now or should I consider another treatment options.?

Radium 223 is treatment used for multiple bone metastasizes in diverse areas where external bean radiation is not warranted. If this applies to you go for it.
I have been on 3 month xgeva for over 2 years and it has been a godsend in keeping the Mets off the bone. I did bring up the very frequent PSA and the oncologist agreed it has been to many, my neutrophils and white cell count wac low 5 days ago but came roaring back today before the infusion and only lab was the CBC, no others. Thank you for your advice and input. The follow up appointments and labs are being done the day before the infusion, finally they listened
My husband's situation is almost identical to yours - PSA 17 and tending upward, 7 mets on his bones, but no pain as yet. His MO has him continuing Xtandi and Lupron and he added Radium 223. He has had his first infusion without a single issue from that so far. I'll post updates eventually.
