Approximately how long can I get with abiraterone plus Zoladex.

I got 5 years with Zoladex alone. (Chemotherapy and radiation treatments as well at diagnosis) Now I am CRPC and have added abiraterone. Approximately how long can I get with this combination. When this fails what can I expect. I know I had increased bone pain before the addition of the abiraterone. What treatment options are there for me in the future.

Hello,

I have answered you before and I am fixing to answer you again. The short answer to your question is nobody knows. I too was on Zoladex (goserelin) for nearly 4.5 years before it started to fail. I added Xtandi (enzalutamide) to the mix and ) I got another five years. Total nine and a half years. I know I have done exceptionally well and many others have done much more poorly. Age, general health, diet, support all factor into the equation. Good luck!

Thanks for the reply again. I am only reaching out here because I don’t want to miss any other treatment options. I have been working full time and I have been playing catch up with my treatments.

Are you still using Xtandi or have you had to try another treatment.

Hello,

A good question, a very good question. Recently Xtandi has started to fail. Not wanting to go on chemotherapy, I applied to the BC Cancer Agency to be put on the SPLASH trial and was accepted. Very humbling experience when one considers only 400 participants world wide and only ten from BC. I am truly gob-smacked! This trial involves Lutetium a drug I have long been interested in. As perhaps you know in September Health Canada approved this drug for use in Canada. The problem is new drugs are funded by individual provinces. I heard if you want to pay for it yourself the cost is over $100,000.00 per treatment. That would be half a million dollars for four infusions. Certainly beyond my ability to pay. With the SPLASH trial I may (depending which arm I am on) get the four treatments for free. Will find out November 17 which arm. If on the SOC arm I will be switching to Zytiga a move that is not currently permitted in BC. Hope that answers your question.

It's not $100K per shot, it's that for the treatment which is four shots I believe.

These are the results of the RCT 3 COU-AA-302 showing that abiraterone offers a survival advantage in mCRPC

Most probable median overall survival will be better now since there are other therapies available (xofigo, Lu 177 PSMA, olaparib, rucaparib, keytruda, cabazitaxel etc) which could be used when abiraterone fails.

I have already had chemotherapy. Do you think this will have any effect on the duration this treatment.

I am not aware of a study done in patients who received chemo before starting abiraterone. I'll try to look it up.
