Decipher Prostate Biopsy genomic classifier to identify risk of metastasis within five years

I am 84 (walk a mile a day with small weights) and in good health except for recently diagnosed with High Risk Localized Prostate Cancer with a Gleason score of 8. In ten days I will have completed 45 days of IMRT radiation therapy. I take a monthly shot of Firmagon. They also recommended Zytiga and Prednisone, 1000 mg on an empty stomach followed by two 5mg of Prednisone daily. Reading about these two, there are recommendations for 250mg to 1000mg Zytiga and with meals or without. It's enough to drive one crazy. After much thought, I decided to go to 500mg Zytiga and one 5mg Prednisone daily. Question: I just came across the Decipher Prostate Biopsy test which is used for patients following a cancer diagnosis to inform whether the patient is a candidate foe active surveillance, needs mono therapy or may benefit from intensified treatment. Do any doctors on this site have any experience or thoughts about this test? The company that offers this alternative to the Gleason score is Veracity. This company attempts to provide clarity and confidence in treatment planning; Lord knows what we have now isI a joke! I would love to know if this company and their Decipher Prostate test is the real deal or just another well meaning failure.....

Before abiraterone became available as a low cost generic drug ($187/mo through GoodRx at Rite Aid), it made sense to reduce the cost by taking a lower dose with food. Now it no longer makes sense.

You should only be taking one 5mg pill of prednisone every day, not two. It replaces the cortisol that the abiraterone takes away.

You are definitely not a candidate for Active Surveillance. However if you are low risk on a Decipher test, you may decide that the abiraterone is unnecessary. It's an expensive test if your insurance won't cover it.

Hello. I had a dechiper test 12 months ago after my rp. I researched this in depth and found that they have ten years of data results and have found this test to be extremely accurate in determining your risk for recurance . Insurance company’s are now starting to cover the cost. My results were .51 which put me in the low intermediate range. The oncologist said no further treatment needed based on results. He explained that this test helps with over treatment that’s unnessesary . Best of luck to you.
