After 6 isotopes of pluvicto my husband had very little side effects . His psa remains low at 1.5 but slowly rising. Has anyone taken six months off from pluvicto and started treatment again?

Maybe is time to get a repeat PSMA scan plus an FDG or Axumin scan to see if remaining cancer is PSMA avid and concordant with any non-PSMA avid sites. If so then further Pluvicto treatment may be helpful. Otherwise probably not. Would consider changing it up with a different systemic treatment.

If he still has exclusively bone metastasis, then Xofigo should be discussed. Is he on a bone-protecting regimen such as Xgeva (denosumab) or Zometa?

Has he had Provenge immune treatment? Get it if you can.

Xofigo is indicated if he has bobe pain. Otherwise Xofigo is not a systemic treatment and it will not be helpful with his cancer on the systemic level. He should focus on systemic treatments only.

I think you can only have a very limited number of these Lutetium PSMA therapy. If he has a life expectancy of more than 8 years Lutetium PSMA therapy should be avoided because of the accumulating long term damage caused by radiation side effects. Be very careful with this therapy.

This is an interesting comment. While at Dana Farber last week I had this same discussion. They are studying the feasibility of using LuPSMA177 again if and when the cancer returns. Obviously this is not an official statement but indicates all aspects of this therapy that are being considered.
