Hello , I came across this web page while searching for options for my dad . 73 yr old with metastatic prostate cancer . He has brca1 gene . Zytiga and xtandi both stopped responding , he has been on olaparib for 6 months . Recent scan showed progression AND rise is psa. What should be next step . Hello and welcome to the group. Can you tell us a little bit more about your dad diagnosis such as initial PSA, if he has bone metastasis , how long was he in ADT ?
I've been on Lymparza for about 4mos now. I "feel" as good as I've felt in years.Extreme fatigue. My PSA has risen from around 16 to now 41.5. CT scan clear, lymph nodes smaller. We have no idea why it is increasing.
Getting another body scan soon. I have mets throughout my skeleton. Less now after ADT treatments.
Hope we find where it's treatable.

Many clinical trials require chemo first.
Novartis is currently recruiting for Lu-177 trials in several states (and in several foreign countries): 'clinicaltrials.gov/ct2/show...
At first glance your dad might be eligible, as it seems open to men who have NOT had prior chemo but are metastatic and have progressed after failing ADT and at least one other agent. There is a mention of BRCA as a possible exclusion, but perhaps because your dad is not responding to olaparib he could be eligible? Worth checking out, anyway. Best of luck!

I had about the same situation. I have BRCA 1 also. My next step after olaparib was chemo. I wish you dad so much success.
I have had ten rounds of docetaxel (taxotere). It has been rough on the last few. I live so far from Duke Cancer Center where I am under the care of Dr. Daniel George, oncologist that they do not usually consider me. Also I have a background of Crohns disease of the large and small intestines and Ankylosing Spondylitis (a form of arthritis of the spine). These things usually eliminate me from any clinical trials and even from treatments. Right now I am at a crossroads and will see the doctor this week. More chemo is one option for me. I have very few options left. Do every option you can.

My Dad is 74 and also has advanced prostate cancer.
His initial response was Docetaxel, chemo, for six rounds. It killed a ton of his cancer!
The first rounds of chemo treatment were not too bad for him. The last two bothered him more, but he toughed it out. He also worked out the whole time and ate as much non processed food as possible, both of which helped.
His psa number started at like 8,000 and has been reduced to .3 and moved up slightly after two years to .6.
I would strongly recommend the Taxotere chemo therapy!

No bone mets. He is very active and that helps with bone mets, imho. He had an almost blockage by the bladder and lymph node involvement in the groin, though. The Docetaxel shrunk the heck out of it! On a side note, he does not have BRAca gene. Not sure if that matters, though.
