Next step- Zytiga vs Xtandi?

Husband finished six rounds of docetaxal in December. Brought PSA down to 0.7 (from 350). Bone Mets have stopped progressing and show “scarring” or “healing”. Going in next week for follow up with MO. I would like to suggest next step or at least be informed with what MO has to suggest.

What is the difference with Zytiga and Xtandi, and possibly Jevtana?

He is still castrate naive and getting Lupron injections every 3 months.

I got nearly 2 years out of zytiga with high blood pressure as a side. Maybe 6 months out of xtandi with bad fatigue. Depends on the person. Good luck!

You have to wait until PSA rises or metastases grow. Then either one is good. Maybe a slight preference for Zytiga because Zytiga->Xtandi seems to forestall progression longer than Xtandi->Zytiga. But other possibilities include Provenge and Xofigo.

His PSA was .7 then one week later .8. Seems amazing numbers to me considering what they came from. But…looks like a rise to me. We’ll find out more next week. Every lab day is scary. Thank you for your advise as always.

My husband took Zytiga along with 10 mg of Prednisone daily. It was effective in lowering his PSA as well as giving him more energy (possibly due to the Prednisone). He said it was the first time he felt better since completing 6 rounds of docataxal. The only side effect from the Zytiga was diarrhea within the first hour of taking the Zytiga. Regarding the difference, our oncologist said Zytiga was less toxic than Xtandi.

I’m interested in this so sorry I can’t help but I am due to add Abi ( plus prednisolone ) to make my triplet shortly ( I have opted to seek Lu-177 to replace the chemo and my adt is Degarelix but like you am hormone sensitive£

My Lu-177 onco had thrown a curved ball by saying he is no sure yet it’s worth adding ( PSA down to 0.7 dropping every 12 days so far) but he’d prefer Enzo as worries about long term prednisolone in a 58 year old. Lovely to have him worry about ‘long term’ to be honest after last year mets progression!

Do now waiting to discuss with MO and realise I am off SOC so no data on my position)

Zytiga and xtandi are different. Failing one does not mean the other will not work. I have had good luck starting with zytiga.

Hi; I only took Zytiga, Prednisone, and hormone injections for two years. Have become stabilized for the past two years. It worked for me.
